Who is providing the service

Elevated Wellness Me Coach Network is a marketplace and communication platform operated by Elevated Wellness Me, LLC. A coach shown in the directory is an independent business and is not an employee, agent, medical provider, or representative of Elevated Wellness Me, LLC. The coach is responsible for the coaching services the client chooses to purchase.

Opening a conversation does not itself purchase, schedule, or begin coaching. Before paid services begin, the client should receive the coach's service description, price, cancellation terms, qualifications, scope, and any service-specific consent that applies.

Coaching scope and client choice

Health, wellness, life, functional nutrition, movement, and habit coaching are collaborative educational services. Coaching may support client-selected goals, reflection, accountability, routines, general wellness education, and implementation of guidance already provided by appropriately licensed professionals.

Coaching is not medicine, dietetics, medical nutrition therapy, physical therapy, psychotherapy, diagnosis, treatment, laboratory or imaging interpretation, prescribing, or emergency care. A coach must not direct a client to start, stop, or change medication, treatment, a therapeutic diet, supplements, or clinician-recommended care. The client remains responsible for personal decisions and should consult an appropriately licensed healthcare professional for symptoms, diagnoses, testing, medication, supplements, exercise safety, pregnancy, injury, or other clinical concerns.

Private messages and records

Messages are intended only for the client and the selected coach. They are not monitored continuously and must not be used for emergencies. The client agrees not to place diagnoses, medical records, passwords, card numbers, or unnecessary sensitive information in messages. Authorized platform personnel may access limited information only when reasonably necessary for security, support, legal compliance, or investigation of a reported violation.

Coach Ledger sharing is separate and optional. Nothing is shared with Coach Ledger unless the client makes a distinct, revocable sharing choice. A coach's private notes are not automatically shared with the client or Coach Network.

Assumption of risk and release

The client understands that coaching, online communication, in-person activities, movement awareness, and reliance on educational information involve uncertainty and may not produce a desired result. The client voluntarily chooses whether and how to participate, may decline an activity, and will stop and seek appropriate help for pain, dizziness, shortness of breath, worsening symptoms, emotional crisis, or another concerning response.

To the fullest extent permitted by law, the client assumes ordinary risks of voluntary participation and releases Elevated Wellness Me, LLC and its trade names, owners, officers, employees, and platform contractors from claims arising solely from those ordinary risks or from the acts or omissions of an independent coach. This release does not apply to gross negligence, reckless or intentional misconduct, fraud, a non-waivable statutory right, or any liability that applicable law does not permit a person to waive.

No guarantees and disputes

The Network does not guarantee a coach's availability, compatibility, statements, credentials, services, results, or continued participation. Public profile review is not medical licensure, an employment relationship, or a guarantee of quality. Clients should independently evaluate whether a coach is appropriate.

A concern should first be sent to support@elevatedwellness.me without including private health details. Ohio law governs platform-related disputes to the extent permitted by law. Nothing in this agreement removes a right that cannot lawfully be waived. Any arbitration, jury-trial waiver, class-action waiver, indemnity, or shortened filing period requires separate, conspicuous terms reviewed by qualified counsel and is not created by this document.

Electronic consent

By typing a full legal name and submitting, the signer confirms they are at least 18, have read this version, had an opportunity to ask questions, intend the typed name as an electronic signature, and voluntarily agree to these terms. The Network records the signer, verified account email, document version, and time. The signer may keep or print a copy.
